Apply for your Alberta Health Card:
At Country Hills Plates, we offer Alberta Health Insurance services.
Whether you are applying for a new health card or need to update your information, we are here to assist you. Here’s what you need to know:
Applying for a New Health Card:
If you are new to Canada and arriving directly to Alberta, your Alberta Health coverage will begin immediately.
If you are moving from another province, there is a three-month waiting period before your Alberta Health coverage starts. You can apply during this period, and the effective date will be the first day of the third month after your arrival in Alberta.
Adding Dependents or Spouse:
All immediate family members (married partners and dependent children) must be included on the same account. If your spouse or parent is already covered under Alberta Health, you will add yourself to their account.
Updating Your Health Account:
Keep your contact information up-to-date with Alberta Health Services. Notify us of any changes in name, address, marital status, etc.
Lost Alberta Health Cards:
To get a replacement card, call Alberta Health directly at 780 427-1432 or write to them at P.O. Box 1360 Station Main, Edmonton, AB T5J 2N3.
Required Documents:
Bring valid and fully legible photo ID (passport, driver’s licence, PR card, etc.), proof of legal status in Canada (birth certificate, PR card, passport, etc.), and proof of Alberta residency (Alberta driver’s licence, utility statement, etc.).
We process applications within three working days. If you have any urgent needs or questions, feel free to contact us at 403-531-6575. Please note, Alberta Health no longer provides temporary cards, and the official card is mailed directly to the applicant.
